Do I have STIs?

For about six months now, I wake up when my foreskin is red and oozing pus. I have taken antibiotics without any improvement. Is my wife giving me infections? Moses

Dear Moses,
When the foreskin is inflamed (posthitis), it may look red and may produce pus or what looks like pus depending on the cause of the problem. Most times inflammation of the foreskin occurs together with that of the penile head (balanitis) hence balanoposthitis.

Usually, poor foreskin hygiene causes your kind of problem with smegma (a lubricant under the foreskin) if not properly washed off, accumulating and looking like pus. To mitigate this, wash the penis with the foreskin retracted. One, however, should not over-wash because both inadequate cleansing and too much cleansing can worsen this problem. Men with a tight foreskin, invalids and the mentally challenged may get problems doing proper foreskin hygiene.
Also, irritation from soaps used while showering at night can lead to your problem and here changing to another soap (unperfumed but not bar soap) can help.

The irritation may sometimes be from latex condoms. So, if you have to use condoms, please change to another type say polyurethane condoms. Mere rubbing or scratching at night can also irritate the foreskin. Skin conditions such as eczema or psoriasis can be mistaken for infections of the foreskin. Posthitis, especially if it also involves the head of the penis may be due to fungal infections, including candida germs looking for free sugar if you are diabetic.
Managing diabetes better may offer a solution. Candida germs may also attack one with advanced HIV infection, on drugs that interfere with immunity including prednisone/dexamethasone used to treat allergies.

Much as non-infectious causes can lead to your kind of problem, today we have infections that are resistant to many of the commonly used antibiotics, requiring taking a swab from the said pus growing the germs and subjecting them to various antibiotics (culture and sensitivity) with the most effective being used.
Infections such as chlamydia may not be cultured here and may not respond to the usual antibiotics as well. Before you blame your wife, please go for circumcision since this is likely to solve your problem once and for all.
